Certificate of Insurance Requirements

Before a contract is signed, external users doing business with the Maricopa County Community College District are required to provide a certificate of insurance that covers either the exact date of their event, or an entire fiscal year. The certificate must contain the following:

  • MCCCD named as certificate holder:
    • Maricopa Community Colleges
    • Scottsdale Community College
    • 9000 East Chaparral Road
    • Scottsdale, AZ 85256
  • MCCCD /SRPMIC named as an additional insured:
    • Maricopa County Community College District, its agents, officers, officials, employees, and volunteers and Salt River Pima Maricopa Indian Community are hereby named as additional insured as their interests may appear.
  • Waiver of Subrogation Endorsement: GL, AL and WC
    • The insurance policies shall contain a waiver of subrogation endorsement, as required by this written Agreement, in favor of Maricopa County Community College District, its agents, officers, officials, employees, and volunteers and Salt River Pima Maricopa Indian Community for losses arising from work performed by or on behalf of the User.
  • General Liability Required. Coverage amounts below:
    • Each Occurrence $1,000,000
    • Damage to Rented Property $50,000
    • Personal and Advertising Injury $1,000,000
    • General Aggregate $2,000,000
    • Products - Completed Operations Aggregate $1,000,000
  • Auto Liability:
    • Commercial Auto Liability must be endorsed to Maricopa County Community College District, its agents, officers, officials, employees, and volunteers and Salt River Pima Maricopa Indian Community are hereby named as additional insured as their interests may appear. Amount is $1mil with a combined single limit for bodily injury and property damage on vendor's automobiles.
  • Workman's Compensation:
    • Workers’ Compensation (if applicable) insurance with limits statutorily required by any Federal or state law and Employer’s Liability insurance of not less than $1,000,000 for each accident, $1,000,000 disease for each employee, and $1,000,000 disease policy limit.
