Financial Aid Forms

This is a list of financial aid forms by aid year. For a current list of items being requested from you, please check for outstanding To Do List items in your Student Center Tasks tile. Make sure each form contains all of the required information and signatures, submit any other required documentation as requested. Please make sure to include your Student ID on all forms and documents submitted.

2026-2027 Financial Aid Forms

Fall 2026 – Spring 2027 – Summer 2027

The U.S. Department of Education recently finalized its federal student aid processing rules. While colleges nationwide awaited these final guidelines, a temporary delay occurred in processing 2026–2027 FAFSA information. 

As a result, financial aid processing for 2026-2027 is currently delayed at SCC.

What You Need to Know

  • Apply now: Submit your 2026–2027 FAFSA application. Being able to complete the FAFSA is not delayed. 
  • Register today: Fall 2026 registration is officially open, enroll in classes before receiving your financial aid offer.
  • Stay updated: Monitor your student email account. SCC will email your student account when financial aid forms and awarding processing begins.

Financial aid Verification forms are now available through our new electronic forms system in your Student Center.

To Access Your Required Forms:

  1. Log in to your Student Center using your MEID and password, and check the dropdown at the upper left to ensure you are on the Student Center homepage
  2. Click on My Student Forms
  3. Click on Financial Aid Forms
  4. From there, you will be able to view and complete any required Financial Aid forms

Required Forms - The following is a list of forms that are only available in the Student Center and available if it is required to be completed.

  • Citizenship Affirmation
  • Homeless Independent Request
  • Loan Discharge Due to Disability
  • Maximum Time Frame (MTF) Appeal
  • Parental Data Override
  • Satisfactory Academic Progress (SAP) Appeal
  • Unusual Enrollment History Appeal
  • Unusual Enrollment History Reinstatement Request

Optional Forms - The following is a list of forms that are only available in the Student Center and are not required. They should only be completed if a student meets the criteria for the request and wants to submit for review.
