Students will use this form to request an Enrollment Verification letter for loan deferments, employment and/or insurance purposes. This form is also used to request a letter of non-attendance or withdrawal from the college.

Students may use this form to submit government issued photo ID for admissions and proof of lawful presence for tuition purposes.

Students may use this form to update legal/primary names or request legal sex changes. A document supporting the change is required.

Complete this form when a sudden emergency or severe change in personal circumstances, or serious illness or injury, results in an inability for the student to continue/complete courses. *This form takes 4-6 weeks to process.

You don't need to send an official transcript to SCC to take a transfer course. Once course prerequisites are verified, we can provide permissions or assist with course registration. You must use your meid@maricopa & password to access this form.

Students under the age of 18 wanting to take college classes while still in high school must complete this form. Parent/guardian consent is required.

All students under 18 except foster youth, must have parent/guardian consent as a proof of financial responsibility and approval of text messaging and automated communication. Find the form in the Student Center-My Student Forms-High School and U18 Forms.

Students who are eligible for tuition rates under this program must submit this form along with a copy of their current State Driver’s License.
