If you have encountered difficulty regarding customer service in one of our Student Affairs departments, or have a concern you would like to share, please use this form to provide us with information and a dean or director will look into your concern. Please also let us know the resolution you are seeking.
If your concern/complaint relates to an instructional department, please contact the department directly. Please be specific in documenting your concerns; they will be treated discretely and confidentially.
