For a Physical Disability, the student must submit a current (within 3-5 years) diagnostic report of any physical disabilities that are based on appropriate diagnostic evaluations administered by trained and qualified professionals (medical doctors, ophthalmologists, neuropsychologists, audiologists). Disability diagnosis categories include: orthopedic disability, blind or visual impairment, deaf or hard-of-hearing, traumatic brain injury (TBI), other health-related/systematic disabilities.

For your convenience, attached is a packet that can be brought to the doctor to be filled out. This packet, once complete, will provide the DRS Director/Advisor with the necessary documentation in order to provide appropriate accommodations. (Medical Disability Packet, Hearing Disability Packet, Visual Disability Packet)
*Disability Resources and Services also helps students experiencing temporary impairments. Each case is considered individually and the documentation requirements are generally the same as those listed above.
