Service-Learning 101 -- What to do next
¨ ¨ Speak with instructor about the service-learning service project for your course
¨ Learn about placement options, guidelines, and course requirements. Student Guide and Resource Guide and forms are located on this website and downloaded
¨ ¨ Complete and sign Experiential Education Assumption of Risk & Release of Liability Form. This form is a requirement of the Maricopa Community College District.
¨ ¨ Research the agency using the agency website listed in the Community Resource Guide to check for agency?s requirements.
¨ ¨ Contact the agency you selected using the SCC telephone protocol guide found on page 5 or fill out the agency?s online application.
¨ Complete the Placement Confirmation Form and obtain all signatures. This form will become the official record of expectations, your hours and other important information for your service-learning experience. Return this form to you instructor or Service-Learning Coordinator, Margaret Daniels, LB-105A (phone: 480-423-6543)
¨ Use the Service Learner Activity and Contact/Time Log. The agency may have a similar log document that you may be required to use. Be faithful in filling out this document. This form will document the dates you served, the activity and the total hours that you put in.
¨ Keep a reflective journal about your service-learning experience and relate it to your course. Your instructor may want you to write about your service experience or give an oral presentation by relating it to what you are learning in the course.
¨ If required by by your instructor, give to your agency supervisor the Student Performance Evaluation by Site Supervisor Form -- turn in to your instructor
¨ Complete the form Student Evaluation of Your Service-learning Experience Form. Return this form to you instructor or Service-Learning Coordinator, Margaret Daniels, LB-105A (phone: 480-423-6543)
