FORMS

The following forms are available for faculty, students, and site supervisors.

STUDENT FORMS 

Assumption of Risk & Release of Liability Agreement (Required)                                   (NOTE: This form is a requirement of the Maricopa Community College District before doing service-learning. Student must read and understand the document before signing.)

Placement Conformation Form (Required)                                                                    (NOTE: This form provides the Service-Learning Program with accurate information about your Service-Learning placement. On this form, you are required to select your placement site from the list of pre-approved Community Partners that is provided by your instructor or in the Community Connections Guide.  This form becomes the official record of your Service-Learning placement for the semester.)

Activity & Time Log Form                                                                                              (NOTE: This form is provided to assist you in tracking the activities and number of hours you serve at your placement site. Check with your instructor and site supervisor to find out if they will be requiring you to turn in a completed time sheet at the end of your service experience.)

Student Evaluation of Experience Form (Required)                                                     (NOTE: This form gives you the opportunity to evaluate the effectiveness of your Service-Learning experience. The feedback that you provide on this form will assist the Service-Learning Program in improving the Service-Learning program at SCC. Completing this form is mandatory for all Service-Learning students.)

AGENCY FORMS

Experiential Education Partnership                                                                                 (NOTE: Experiential Education Partnership Agreements are universal, for all colleges. New Agreements are required for all Community Partners not currently listed in this database.)

Community Partners Information Sheet(Required)                                                     (NOTE: Submit the required Experiential Education Partnership Agreement, Community Partners Information Sheet, and a copy of your proof of Insurance/Liability. Please mail all required documents to:  Scottsdale Community College, Office of Service Learning, 9000 E. Chaparral Rd., Scottsdale, AZ  85256)                                                                                                                                           

Performance Evaluation by Site Supervisor                                                              (NOTE: This form is optional depending on service length and desire of the instructor. The student is responsible for making and distributing copies to their site supervisor and for his/her own records.)
