  Print this Form and Attach:
 
 
  1. A current course outline that includes: course objectives, expected learning outcomes, primary content and concepts.
  2. A preliminary needs assessment, conducted with your Department Chair that includes answers to the following questions:
    • To whom is this course targeted?
    • How will it impact current course enrollments?
    • Is a section of this course currently available online or in a hybrid format?
  3. A description of the evaluation and assessment processes that you will use to ensure course quality and effectiveness.

Student Survey in Blackboard

For an online course, be sure to include, as one of the evaluation processes, a Student Survey. 
Follow these steps to import a sample survey into your course:

1.       Go here:  http://www.scottsdalecc.edu/scconline/using_blackboard.html.

2.       From that page, click on ?Student Survey? link and choose to save this ZIP file to your computer?s desktop (or another desired location).

3.       From within the control panel for your Blackboard course, click ?Survey Manager,? then Import button.

4.       Browse to find and select the ZIP file you downloaded in the second step.

5.       Click Submit.  May take awhile, but eventually the Survey Import Complete page appears.  Click OK.

6.       To revise the survey, click the Modify button to the right.
