Effective immediately, applicants need to provide proof of ALL prerequisites before you get a registration signature. This includes: 1) Fingerprint Clearance Card, 2) CPR card, 3) Proof of basic reading score, 4) Proof of 2 MMR injections, 5) Proof of 2 Varicella injections, and 6) Results from TB Skin Test #1 and TB Skin Test #2.
