If a qualified student is receiving academic accommodations in an instructor's class then that student will need to provide the instructor with an Authorized Academic Accommodations form.  If a student does not provide an istructor with one of these forms, then the instructor is not required to provide any academic accommodations.  To see what one of these forms looks like, please contact the Director of the DRS office.
