Cooperative Education Forms

Enrollment Contract Form is completed in the Cooperative Education office, SC131 & SC132. The student is then registered for Cooperative Education credits and is billed for the credits by SCC's Admission office.

Training Agreement Form will be given to the student after the Enrollment Contract Form is filled out.

Learning Objective Form will be brought by the Faculty-Coordinator to the first on-site visit for completion.

Employee Evaluation Form will be sent to the student by the Co-op office. Student is to give the form to the employer for completion.

Student Final Report Form is the outline to be followed when summarizing the work experience. The Evaluation of Co-op Education Experience form is to be completed and attached to the Final Report.
