1.  Download an Application Form and a Reference Form.  Submit completed application by February 20, 2009 to: Jen Sydow, SC-125, 480.423.6724
3.  Complete interview.
4.  Once accepted, make program payments according to the deadlines:
        - First deposit due: March 13, 2009
        - Final Balance due: April 17, 2009
        ** If a student cancels after March 27, 2009, a refund is only available for the tuition cost, registration fee and course fee.
5.  Attend the mandatory orientation that will be held Saturday, May 2, 2009 (9am-1pm).
